    • Copy of State of Iowa Retail Sales Tax Permit
    • 1,000,000 Automobile Liability Insurance
    • 1,000,000 Commercial /Business Liability Insurance
    • Please include any social channel names & website URL
    • A photo of your truck as it appears today
    • Copy of appropriate state-issued mobile food unit license
    • Copy of Certificate of Food Protections Manager(s) certifications
    • (If applicable) Copy of appropriate food establishment license issues by Iowa Dept of Inspections and Appeals for any commissary kitchen or other premises where food is prepared for sale from the mobile vendor unit
